Peritoneal Mesothelioma: Causes, Symptoms and Treatments

Peritoneal mesothelioma is a rare form of a rare type of cancer. Mesothelioma is a cancer that's caused from exposure to asbestos. Less than three thousand people are diagnosed with this type of cancer in the US each year. Of those, between ten and twenty percent have peritoneal mesothelioma.

Peritoneal mesothelioma is an aggressive cancer that grows in the peritoneum, which is a layer of tissue that essentially covers the abdominal area. Other types of mesothelioma form in the tissue that surrounds the organs like the heart and lungs.

This cancer is caused by inhaling asbestos. The fibers don't break down in the body like many other ingest or inhaled substances. They never leave once inhaled, and lodge between mesothelial cells, which gives this type of cancer its name.

Because today we know that asbestos is dangerous and deadly, it has to be removed from public buildings and we're warned about having it removed from our homes. Special hazardous materials handlers, known as hazmat workers, have to remove asbestos while wearing protective suits and using special equipment to keep it from getting on their skin or into the air.

Peritoneal mesothelioma, like other forms of asbestos-caused cancer, will often not show up for several years after the initial asbestos exposure. It can even take 2 to 5 decades before symptoms appear. But once the exposure happens, and the asbestos fibers lodge between the cells, they cause irritation and inflammation. This causes the cells to grow abnormally and they become cancerous. As the cells continue to grow and divide in this warped way, the peritoneum thickens, and the cells eventually form a tumor or often several tumors.

Symptoms can include things like fever, sweating at night, diarrhea, constipation, stomach pain, stomach swelling, weight loss, nausea, vomiting, anemia, constant fatigue, and in many cases the lumps growing on the abdominal wall can be felt beneath the skin. Since most of the symptoms are symptoms of a variety of ailments, having them does not mean you have peritoneal mesothelioma. X-rays can confirm or help rule out the disease, as can other testing like tissue or fluid biopsies.

This type of cancer has often been present in the body so long before it's discovered, that it's typically very hard to treat. By the time it's discovered, it's almost always last stage cancer, which means it has spread over the affected area and often beyond it. Surgery can be performed to remove the obvious cancer, and then chemotherapy and radiation are used to kill any remaining cancer cells or at least slow its growth. There is no cure, but these measures can bring temporary relief from the pain of the cancer and prolong the patient's life.

A chemotherapy treatment known as intraperitoneal chemotherapy can sometimes be used to help fight the cancer. In this treatment, the medicine is injected into the peritoneum instead of the bloodstream, to fight the cancer at its source. This treatment and clinical trials of new treatments often give peritoneal mesothelioma patients the best chance at longer more comfortable lives than standard treatments alone.

Pleural mesothelioma is the commonest type of mesothelioma. Mesothelioma is a rare form of cancer that attacks the mesothelial cells in the mesothelium, the tissue that forms the membranous lining of the body's major internal organs and cavities.

Pleural mesothelioma develops when the cancer affects the pleura {the membranous lining of the lungs}.This form of cancer is commonest in the UK, followed by South Africa, then Australia, then USA, then Eastern Europe.

The pleura has two layers which gives support and protection to the lungs and chest cavity:

-The outer layer, or parietal layer, lines the entire chest cavity and the diaphragm.

-The inner layer or the visceral layer which applies directly over the lungs.

Pleural mesothelioma can start from any of these two layers then spread to the other layer. Exposure to asbestos has been strongly linked to the development of this type of cancer. The toxic fibers get trapped in the spaces between the mesothelial cells of the pleura.

Symptoms

The asbestos fibers trapped in the body cause cancerous cells to multiply and divide haphazardly without control. These results in the thickening of the pleural membrane and mesothelial cells, leading to fluid build up in the pleura {pleural effusion}. This accumulated fluid will gradually start pressing on the lungs and the respiratory system leading abnormal and difficult breathing.

These developments eventually form the basis of most of the symptoms of pleural mesothelioma. These symptoms include:

A} Persistent dry or raspy cough

B} Haemoptysis, the coughing up of blood in the sputum

C} Difficulty in swallowing {dysphagia}

D} Difficulty in breathing, labored breathing known as dyspnoea

E} Pleurisy, pain in the chest during breathing

F} Development of lumps under the skin of the chest wall

G} Night sweats or fever

H} Unexplained weight loss

I} Fatigue

Diagnosis

Just like in other forms of mesothelioma, the symptoms of pleural mesothelioma show up many years after the initial exposure to asbestos and these symptoms are not specific to pleural mesothelioma, they also occur in some other less serious lung diseases like pneumonia and influenza.

All these factors make the diagnosis of pleura mesothelioma very difficult.

Once there is a strong clinical suspicion of pleural mesothelioma, diagnosis is then confirmed by imaging tests such as x-rays, CT scan, to confirm the location of the cancer. The patient is also subjected to fluid and tissue analysis known as biopsy to confirm the type of cancer involved.

Treatment

Newly developed drugs and detection techniques have improved the outlook for patients with pleural mesothelioma.

In general there are three main modes of treatment: a} Surgery b} Chemotherapy c} Radiotherapy. Most patients receive a combination of two or more of the different modes of treatment.

The earlier the cancer is detected, the better the prognosis and the wider the range of treatment options. When the tumor is detected early, curative surgery is done to remove the cancer, this is followed by chemotherapy or and radiotherapy to kill off any remnant cancerous cells. Curative surgeries is only carried out in patients that have good general state of health and whose cancers have not spread beyond localized sections of the lungs.

The level of progression of the tumor is classified in terms of the stage of the cancer. The stages range from stage 1 to stage 4 depending on the level of spread of the tumor.

Stage 1 and Stage 2 pleural mesothelioma have better prognosis and better treatment options.

The latter stages of the cancer, namely stage 3 and stage 4 are mostly limited to palliative modes of treatment to improve the quality of life and not their prognosis. These palliative treatments include the draining of excessive fluids in the pleural spaces and surgical removal of tumors to relieve pressure on the lungs.

According to current studies, approximately 10% of all pleural mesothelioma patients will survive for three to five years after diagnosis and 5% will survive for 5 years and above.

Is it inevitable that all people who worked with asbestos will get Mesothelioma? What are the symptoms?

They are reconstructing my school, and they have to redo certain rooms because there is asbestos in the walls, roof and floor.

i was told that once they start tearing down these places it becomes active and becomes a carcinogen.

They only have a thin piece of plastic and duct tape (the plastic has holes in it too) covering the entrances to these places.
